Audit trail and receipts

The append-only record behind every decision and change.

Every change to the record and every runtime decision in CognaOne is written to an append-only trail. Nothing is deleted and nothing is overwritten, so any decision can be reconstructed later.

What is recorded#

Conflicts	Detected, resolved (with the resolution), deferred
Decisions	Action evaluated, verdict, which requirements passed or failed
Policies	Created or changed, violations detected
AI systems	Registered, changed, roles assigned and ended, lifecycle moves, imports and rollbacks
People	Approvals, resolutions and assistant approvals

Trace bundles#

Each event carries what happened, when, who or what triggered it, why (the causal chain), and links back to the evidence it came from.
