An action was blocked

Every block names what failed: a missing key, a conflict or a policy rule.

Record the required key, or confirm the item if it is still in formation.

Resolve the conflict on the key; see Conflicts not resolving if it stays open.

Obtain the approval, or check the value against the threshold.

Send a new clientRequestId to evaluate against the current state. A retry with the same ID replays the original decision unless the state has changed.
